I agree ... not really professional.  Of course, I still cringe when I have to type "belly" instead of stomach.

Well, now if it is a verbatim account and
the dictator said, "during this hospital stay" and the MT transcribed "during this hospitalization" -- that is wrong. It all depends on what the client profile outlines.

It sounds as if he was obviously having trouble organizing his thoughts. This is exactly why I no longer work on verbatim accounts. I like being able to use my brain and be trusted enough to make the necessary corrections.

I to am QA and the only way I would take the and out is if it a verbatim account and the doctor did not dictate it. I don't think QA changes things just to make a change. I know I don't.
Obviously you don't have a verbatim account.
The time when MTs were even permitted to translate jargon and expand abbreviations are behind us.

The poster below who said she shouts it back at the screen gave me a big laugh..Its seems funny.. if we say it, its okay, but don't let a doctor!  I think, "How dare her!" But, I did get a laugh out of it, even though I don't like that word!

The problem is if the account is true verbatim...

I only have one doctor on one account who insists her dictation be transcribed verbatim.  She makes TONS of boo-boos, but I type 'em.  The trouble there is, doctors like that regularly say really wrong and stupid things, we transcribe verbatim, and they say we transcribed it incorrectly. 


I honestly believe some doctors are just so arrogant they can't EVER admit to making a mistake.  I am grateful to have docs who actually thank me for catching their errors.  Unfortunately they aren't all so gracious. 


Sometimes you just can't win. 


Also some companies are caught up in the "no blanks" or "everything must be transcribed" - even if it is totally unintelligible gibberish.  So, reports are sent with GUESSES (by QA) with OUR names (initials) on them.  Or QA changes something (that we transcribed correctly) and again our initials are on it.  Chaps my buns.     Now, QA people (and I QA a bit myself, don't get all uppity.  Not ALL QA is like that.

Verbatim is verbatim, but I defintely to the little things to make the doc look better such as..sm
an eye problem instead of a eye problem.  I definitely wouldn't change too much.  Stick with what they say and only make changes when you absolutely think it is necessary.  I took a test last night and have an interview today and this is what I did.  FYI, I do clinic work.

If it is in quotes, you had better leave it... esp in regard to the legal aspect, but also the integrity of the document.


That said, usually the client has a preference regarding these things.


 


The BOS can cram it. That's C-R-A-M and don't put it in quotes. nm.

Not your call. Transcribe in quotes SM
or if you truly refuse to transcribe that word, use whatever system your company has to make the report go to QA.  There are reasons dictators want a thing like that transcribed--it demonstrates noncompliance or abuse of staff and it's important it be transcribed into the document.
you can use apostrophes to emulate quotes.
Two apostrophes = one quotation mark

Like ''this.'' (4 apostrophes total, 2 in front and 2 behind word)

Just a little trick I figured out a long time ago for this board.
If the doctor dictates it, you transcribe it, in quotes, as this
is what the patient said.
I was told if it is in quotes transcribe it even if curse word...
The doctor wouldn't have said it if he didn't want it in his report. If it wasn't in quotes now no I wouldn't have put it in. But since he was quoting yes I would.
They are teaching you to always put the period inside quotes now. I just took a college English
It still looks wrong to me that way, but that's how they're doing it.

I've never had a problem switching from account to account.
The problem I had was being switched too soon. If I don't get one account spec solidly in my memory with QA feedback on mistakes before starting another, I get confused. The other problem I had with former jobs was inconsistent QA. One person tells you one thing, another person tells you another.

Lately, I don't have a problem with it. Right now, my binder has 8 account specs (some I've never had to use) and I'm picking up a new weekend job for a while. The terminology and report formats stay basically the same. It's just all those little preferences. LOL

That's one thing I really miss about living in AZ, no DST ...... If I could figure out how to move Camelback Mountain to Texas, that's where I'd retire LOL.

We have a show on Sunday nights here, a call-in show that deals with all sorts of issues.  A while back there was a huge ruckus when the Oreck plant relocated to Tennessee, they said they "couldn't find qualified workers" after Katrina.  Before I moved here I guess there was a jeans plant of some sort, Levis or whatever, and it went to Mexico.  So I e-mailed the host of the show and told him he should consider doing a show on outsourcing and detailed my experience at having been outsourced, and the whole health care info going to India thing, and I did not hear back from him!  I was shocked because I interpret no response as not giving a rat's behind about where his info goes.  Maybe he didn't want to "get involved" because the hospital that ditched us is a big advertiser on the station, I don't know, but I sure thought something positive would happen, the topic would come out in the open, but as of this point it doesn't look like it will.  We all need to keep knocking on doors and rattling windows, though, or we'll be sunk.  My opinion and I'm sticking with it.

If this is something that appeals to you, I say go for it; however, I don't think MAs are compensated as well as they should be for their knowledge (sound familiar?).

I became an LVN in 3 semesters and an RN with an AS degree in 4. I'm job hunting for a nursing position now with not much success, even in the midst of this critical nursing shortage, so I can only imagine that the immediate outlook for medical assistants is not that bright.

Saying all this, the MAs I've worked with have been awesome, brilliant people who loved what they were doing and were HAPPY.

I think you should follow your heart.

so far, quite good. I've had the same account since I started. Nice equipment, good tech support (which I've only needed once), and calm staff who leave you alone to work during the day. They're definitely worth considering.
Be sure you ask your senator to vote....sm
to pass the Safe ID act which was presented to congress by  Senator Markey.  This particular bill would require anyone sending things like medical or banking records out of the US to obtain consent from the consumer/patient ahead of time as well as yearly afterwards.  Educate all of your friends, neighbors and family that  HIPAA does not cover  your medical records leaving the US and therefore when they receive medical services they should request in writing that their medical records remain exclusively in the US.

I think she's played the game well. Terry is obnoxious. Danielle does not deserve it. Shane was a bit irritating but he made it interesting to watch though.

I must admit the first few weeks watching him was soooo annoying, but he has grown on me and I actually enjoy watching and listening to him sing and perform. He's good and I think he deserves to win! However, I must say I was confident that Chris would win, but unfortunately not. Therefore, my vote is for Taylor!!!!
