The VPN is how you connect to the hospital you're transcribing for. Do you have what looks like a padlock on your desktop that says cisco vpn by any chance? Before you can even open Extext, you must connect to the VPN which gives you a virtual connection to the hospital and then you open Extext. Or do you access by using an Internet site? Whatever you connected to to upgrade is what you must connect to use Extext. Either a VPN they set you up with, a secure Internet link they set you up, or another VPN, maybe Citrix (which I'm not familiar with).
