I actually had a doctor dictate on a VERBATIM account: "The patient fell off a pallet, and a shovel handle went right up his a**hole, and tore him up pretty good up there." HAVE YOU EVER? Thankfully he later went back during the report and changed it to rectum, but he kept in the part about tearing him up pretty good up there. Sooooo unprofessional. The same doc also once dictated how the baby he was seeing was fussy, and, once he held her, she "took a DUMP" right in his arms and calmed down :)
