How do you get to 200-300 lph?

I'm pretty new at this. I only have 4 months' experience. I've been seeing how some can transcibe at 200-300 lph or more. How long does it take to get to that speed? I have to work all day long just to reach somewhere between 400 to 500 lines. It's embarrassing to admit, but it's the truth. Also, I've been receiving hostile email from QA because of leaving blanks. They say I'm not meeting the expectations of an MT with my experience, but I'm really trying to do the best that I can. What should I do to get better at this?
