Sounds like herpes "syn" rash. Not skin, cause later dates no cutaneous component. sm

The nature of her chest wall spasms is not clear. The first thing that comes to mind is herpes, but this would have to be herpes syn rash. There does not appear to be a cutaneous component to it, and postherpetic neuralgia is less likely but still remains the most likely diagnosis.

S refers to sound: First, second, third and fourth, (S1, S2, S3, and S4).

A2 and P2, the other post mentions, refer to aortic and pulmonic valve closure, A2 for aortic valve closure and P2 for pulmonic valve closure.

I have a doc who is stating in a procedure note that he used what sounds like chloraskin prep.   I cannot verify this anywhere.   Any suggestions as to what he is saying?   TIA!

I had a report where the doc said he prepped and draped using s/l Chloraseptic (very clear).  I left a blank for QA because the only Chloraseptic I know of is a spray or lozenge for a sore throat.  QA filled it in Chloraseptic.  Am I missing something?
