it would be something like patient presents at 7 cm dilation, 50% effaced, with the head at -1, or sometimes just as numbers with slashes 7/50/-1. I'm not positive of the order of the values, though. Maybe someone more experienced with OB notes can help you there. You wouldn't have specific numbers like 6.9 etc., at least not that I've ever heard of. nm

• Insert peelable introducer sheath/dilator assembly over guidewire using a twisting motion. Continue to advance assembly until sheath position will ensure catheter entry into vein. (Figure 2)
• Remove dilator and advance catheter through sheath. (Figure 3)
• Advance catheter until approximately 5cm remain outside sheath.
• Withdraw sheath from vein and pull both tabs to split. Discard sheath.

The gallbladder was elevated with the sac of the cystic duct free initially. It was a relatively long structure.

I need help with this part on a not so great recording.  The doctor says "At the bronchus intermedius is a partially occluded [s/l eep] duct and [s/l polyp or polished] white pale tissue which [bled (I think)] easily."  Can anyone shed any light?  Thanks.
