Patient in for treatment of a skin lesion over the extensor tendon of the right hallux. Continues to have problems with the lesion over the s/l bowstrung tendon.

This patient is a xx year old female with a past medical history of hypertension, status post gastric-bypass surgery in (DATE) who presents for preoperative clearance.  She is undergoing surgery by Dr. XXX on (DATE) for thighplasty and panniculectomy   Apparently, she is feeling well and has no complaints.  Specifically, she denies any chest pain, shortness of breath, dyspnea on exertion, PND, or orthopnea.  She denies any bleeding or clotting disorders.  She has no difficulty with anesthesia.

Pt diagnosed with bacterial skin infection, took Bactrim DS and it started to clear. One bump had purulent discharge, and the nurse is concerned that it is MRSA, or that he is a carrier.

He was s/l not solved/or absolved for MRSA at the clinic. Thanks.

Impression is period fever syndrome. Giving brief trial of cimetidine to see if this will cut off his episodes, suggesting that they are in fact s/l foppa/FOPPA

Then s/l SMF gene testing was ordered. He says this part so fast that I'm not sure there could be more initials.

Patient has a strongly positive ANA, but normal C3 and C4, with a normal CBC and sedimentation rate, but she has a weakly positive rheumatoid factor and a low-titer of s/l anti-phos-photetal serian (maybe serum) antibodies.

PLAN:  We discussed that given the findings on his MRI and given the response to the injection, it is reasonable to proceed with a left hip arthroscopy and labral débridement.  At that time, we will look for any head-neck junction irregularity or (s/L-pencil-) lesion.  He understands the risks and benefits of surgery and wishes to proceed.

MRI demonstrates some degeneration of the medial meniscus.  I do not see a distinct split.  There is some wear on the surface medially.  There is some (s/l-concongral-) change deep, a little cystic change is noted on the inner side of the knee with a little spur at the corner, which could be giving him some of his problem.

Later, MRI showed comminuted extra-articular fracture of distal radius.
