Numbering lists help.

Subject: Numbering lists help.

We have a division on our department.  Some say to number the assessment list with a 1. even if there are no other assessments.  Others say if there are more than one we, number, but if there is only one, it is not a list and you do not put in 1.  Whats the deal folks???

I have been on psychiatric accounts using numbered lists within each axis or commas, and they could be numbered within a paragraph format or on separate lines, sometimes starting on the same line as the axis heading, sometimes starting the line below. It's all up to the facility or doctor. Even the formatting of the numbers is determined by account specifics, i.e. 1)double space vs. 1.double space vs. 1.tab, etc. I would check with your supervisor, the doctor, or QA, depending on your work situation.

numbering with meds

What is the rule with numbering and medications.  This is the sentence:  We used a solution of 2-1/2 cc of 1% lidocaine.  Shouldn't it be 2.5 cc of 1% lidocaine when speaking of meds?

MS-Word help. numbered lists

Wasn't sure which board to post this on. Will try here. I've just reinstalled MS-Word. Previously, I had it set up to create numbered lists as soon as I typed a number, two spaces, and began typing text. After that, every time I hit ENTER, the list would continue numbering. I've gone to the bullets and numbering menu, and I can get the automatic numbered lists turned on. My problem is that every list is indented, but I want the lists to be flush with the margin. I can right click on the list and select "decrease indent", and that moves everything over, but I'd like to get rid of that step. Previously I had it set up to do all of that automatically. I just can't remember how to set that up now.

I hope I described this problem clearly. If you can help me, I'd really appreciate it!

I want to make sure I am correct in how I am transcribing this one doc I have.  He is always saying "He denies cough, congestion, exertional pain, fever, chills OR sweats, weight loss OR hemoptysis."  I've been using "and" between these symptoms as the patient is denying all of them.  "He denies fever, cough AND congestion."  As opposed to "He has no cough, fever, OR chills.  Is this correct?  This is driving me crazy.  Thanks very much.
