I have two medications I cannot find.  One s/l taramasotinate 10 mg daily (not sure if this is supposed to be two separate words).  The other one s/l simkalset 30 mg daily.  No diagnoses are provided.  Thanks in advance!

You are probably right as it being Tessalon Perles, but I would have to flag it as such and send it with a blank since it is not a verbatim account.

the patient wrote him/herself. When they can't tell what it is, they often spell it the way the patient did. I'd leave a blank and make a note with the doctor's spelling if you've already researched the term and found nothing to verify it. I found nothing in my Drake. I never trust a doc's spelling of ANYTHING!!

Recurrent ovarian cancer currently with symptomatic ascites and decreased urine output.  It is likely that the patient is currently "A-THIRD-SPACING" from her ascites and is somewhat intravascularly depleted. We will therefore admit the patient for IV fluid hydration and give her a liter bolus.

Two questions on lung cancer patient report

My questions are in two sentences one after the other.  Sometimes he says period but then really continues on with the same sentence, so I'm not sure if the first sentence is really a complete sentence or not, so the sentence before basically said a PET scan revealed an increase in the size of the hypermetabolic subcarinal lymph node, consistent with subcarinal metastases. 


1.  "Stable [s/l lock laid right] pleural fluid thickening, but the associated hypermetabolism has resolved."  This could maybe be "stable likely right pleural," but I'm not sure if that makes sense and I think I'm hearing a "d" at the end.


2.  "There was a small right level 2 node that has become [s/l non rye valk] consistent with an evolving inflammatory process." 


His baseline is clear. ??clerins is 74. Cancer patient.

We have a division on our department.  Some say to number the assessment list with a 1. even if there are no other assessments.  Others say if there are more than one we, number, but if there is only one, it is not a list and you do not put in 1.  Whats the deal folks???
