 I think you might consider Cozaar, hydrocortisone, Norvasc, Toprol (or is the dictator saying metoprolol), Pravachol, Aranesp.

 Just because they pronounce badly, depend on your drug reference resources.  You have to ignore the bad pronunciation and rely on your resource materials.  It is amantadine just as it says in your drug book

 I had checked my drug book as well as an online reference and did a Google search.  However, I often find that some medications are not in my reference materials and I found spelling for both ways with the Google search.  (Posters often misspell things when posting on the internet).  When I looked up what amantadine was used for, I found that it was an antiviral agent for Influenza or viral infection.  This is not what it was being prescribed for, so I thought just maybe there could be such a drug that sounded similar.  It wouldn't be the first time I have run into this.  Thanks for you input.

 HCT is hydrochlorothiazide and when together, Benicar HCT means hydrocholorthiazide.  It should not have to be spelled out and cannot be mistaken for hydrocortisone because there is no such thing as Benicar hydrocortisone. I would leave as is as your QA and others have stated.  Hopefully this clears it up.

 Insulin is usually the same concentration here in the US with U-100 insulin being 100 units/mL, so I don't think that it would mean 150 units/3 mL or anything like that. I'm not sure what the dictator is trying to say though. Usually a sliding scale is based on actual blood sugar minus target blood sugar divided by the correction factor. For example, my son's correction factor is 1 unit of insulin per 200 mg/dL over the target blood sugar of 120. If his blood sugar was 400, you would subtract 120 to get 280 and then divide that by 200 to get the total units of insulin, in this case 1.4 units. I know that probably doesn't help much with your report, but maybe it will be useful information for you someday.

I have a doctor that dictates the units so quickly that I'm not sure what she wants, i.e., 6 units s/l 60 and 16, 4 sounds like 40, etc.  I'm not familiar with insulin dosages and would like to learn.  Thanks.

 We did her A1c a couple of weeds ago and it was 8.1.  I had asked her to increase her NovoLog to s/l 4 (or 40) units with breakfast, s/l 4 (or 40) units with lunch, and s/l 6 (or 16) units with supper.
