Smart quotes are where they look like commas, first is "upside" down and last "right side up" You can sent them in Word in the autocorrect section under autoformat as you type.

I think it should be - dieting or exercise; he is simply "too busy". The patient is likely just saying I'm too busy....so I just just quote what was probably actually said by the patient. ; )

I'm assuming the "profigmentation" is maybe pigmentation that he just stumbled on but I'm not positive on that.  The patient was being seen for bilateral lower extremity recurrent cellulitis.


Any ideas?
