means ample, ample means "of large or great size." I certainly don't think the prostate was giving out presents (another definition of generous). Get a dictionary. You really need one.

I think it should be - dieting or exercise; he is simply "too busy". The patient is likely just saying I'm too busy....so I just just quote what was probably actually said by the patient. ; )

how I keep it straight

Since the foramen (foramina-pl) is a specific structure, the combining form would be incorrect.

but it DOES depend on the client profile/preference like the other poster said.....I get this format all the time in REAL consult reports (not letters) and I just type *Dear Dr. Smith* - I have just seen your patient....blah blah (can usually go under reason for consult or HPI) and then do my headings as dictated.

hardcore verbatim you have to leave it as is, whether it looks ugly or not.  I know, I really don't like it when they abbreviate left and right, leave words off sentence structures so they can speak faster and get it over with.
