Well, it is neurology with a chief

complaint of unsteadiness of gait. she has a lot of problems, but he says "past medical history: Medical Illnesses: FBS 110. Would you think that would fall under medical illnesses? I'm pretty sure that is what he is saying, though
IMO he and is should not have quotes around them...

I think it should be - dieting or exercise; he is simply "too busy". The patient is likely just saying I'm too busy....so I just just quote what was probably actually said by the patient. ; )

...I hear ya!  Was one of those days for me, too...grrrr!  Had a report today where the doc said pulse ox....all I could hear was something like PSACS...it finally hit me & I felt like such a nincompoop
