The doctor is listing the patient's surgeries so I do not know anything about the following except for what it sounds like: s/l "omar transforisition" on the left side.  Sorry that is all I have.  Thank you in advance for any help.

"A two-ought probe was passed through the upper and lower punctum."  I think "ought" can mean zero, so would it be typed "A 00 probe" ?  It seems like I remember this dictator saying "double oh probe" before.

It sounds clear, but I can't find anything close on Google or in Stedman's.  Any ideas what this needle might be?

I have had that happen to me before, and I was told by QA to not move things around in the report, maybe that is the same for you, but if I were typing this report, I would type it in the order as is dictated, even if that means typing it under the heading PLAN.  Also, his vitals were probably taken at the end of the exam as they were repeated, so it would make sense to type them again under that heading.  Hope that helps!
