I am typing a procedure (verbatim) and the surgeon says that he left a "half centimeter flap." Does anyone know how this is correctly expressed? Should I leave it as half-centimeter or should I go all metric and use 0.5 cm?? Help!!

I would type 0.25 if pertaining to medication or lab values.  I was told never type in fraction form.  So, if I had a doubt, I would spell it out one-fourth.

Thanks. I appreciate it. I had a corrected report and the correction was wrong because it did not have a hyphen. It lowered my score but then that picky QA needs some listen in typing out fractions.

I always type out fractions, i.e., one-quarter, one-half, as it can be confused with 1-2 or 1-4 even if written 1/4, 1/2.

You should not be using fractions with a metric unit. If they say "four-and-a-half cc," that gets transcribed as 4.5 cc (or mL). You can use fractions with non-metric units like inches or days or whatever.

Okay, I've have to say that I never really got the "grain" concept in terms of drug usage and was too lazy to ask my instructors back in the day, and now my lack of knowledge has come back to haunt me.  The doc says this patient is taking "three-fourths of a grain of phenobarbital."  How do I transcribe this? Anyone?

Is one form correct over another when abbreviating the measurement 'microgram?'  I know I've seen it abbreviated as mcg, but in my chemistry background, we always used the symbol that looks like a funny letter 'u.'  I can type that symbol in Word, but I'm not sure if it's necessary or preferred.  This is the first time I've had the term 'microgram' dictated. 


Side note, do you use upper or lower case 'L' to represent a liter?  I've seen both used.

Many, many hospitals use the PACS system and well as imaging centers. It is the electronic digital system that allows the x-rays to be viewed on a computer screen as opposed to the old school view box. They usually have an PACS administrator that oversees all problems related with the system. It now has an added dictation/transcription portion that can be purchased to go along with the system. If you are doing Radiology transcription PACS is a word you will hear alot, not so much in dictation but in film storage, etc.
