I've checked my BOS and googled, but I'm still not sure of the proper way to transcribe this. 


This is a patient with a history of stage III colon cancer, s/l Tc N1 with 2 out of 8 positive mesenteric lymph nodes.


Do I have the Tc N1 part correct?

Guy has colon adenocarcinoma w/ mets to the liver.  My first thought was Xerox, but as I listen more, she may be saying more of an "L",  "Zelox" and Avastin chemotherapy.  I can't come up with anything.  TIA!

Then he states, "The patient was started on cycle #1 of bleomycin, etoposide and TESPLATIN?


 


So may be I am hearing BEP wrong and should be BET? because I am hearing TESPLATIN?


 


The patient has testicular cancer if that helps.

I'm not sure if I'm any were near the right spelling or pronunciation but it is something like pa-nen-ta-meb and Xeloda.


Here's part of the report: Status post left hepatic lobectomy, isolated segment V resection and radiofrequency ablation of five additional lesions with multifocal metastatic colorectal cancer.  She is restarting her penetameb and Xeloda.  she has also had TheraSphere treatment.

You heard it right. It is "1.2 mg IN 12 cc of chemotherapy."

It basically means the patient received 1.2 mg of a particular chemotherapeutic agent (which would be "specific" to her malignancy and which would be documented in her medical record/chart), mixed in 12 ml of a solvent which could be normal saline/dextrose water/or others, so that that particular chemotherapeutic agent that she is on, can be administered to her in the form of an injection.
