My primary acct is a verbatim but am allowed to make grammatical corrections as long as it does not change the meaning. One of my docs dictated *monna releaser*. After getting my laugh for the day, I typed *I am going to release her*....this one was not an ESL. Gotta getta good laugh once in a while to keep sanity....

Would this be actual "verbatim" transcription or correct, per BOS? What would vertaim be when you have to type exactly what they say even if it is not proper? I think that is what the OP was asking. Now I am curious.

PPD is a test for tuberculosis.  You aren't just writing a document to appease the doctor.  You are writing a legal document that many other people may want to read, including an insurance auditor or a lawyer, neither of which may have any clue what PPD means EVEN IN THE CONTEXT OF THE SENTENCE and will have no inclination to look it up.


That's just me, someone who audits physician records and slaps them on the wrists for this like this, but you do want you want.

General surgeon talking about frozen section pathologic exam of squamous cell carcinoma lesion. First frozen section came back with positive margins so he went back to excise a second specimen which he submitted S/L "on fos" with the new margin up. This is a verbatim account and this dictator is on 100% QA (all reports) since he is so SPECIAL. I referenced the other reports in his MD data base and found "en Fos" which would have been put there by QA before the report was signed off to the hospital. I can't find this anywhere and was wondering if anybody has a clue?

When typing verbatim would you commonly use a virgule like that? This verbatim thing is new to me and its got me all turned around. I cant tell if doc is saying "gasenous (meaning gaseous) indigestion" or if he is saying two symptoms the patient had, gassiness and indigestion. He says them right in a row like that.

I personally do not have strict verbatim accounts, so I would not put in the word systolic.  Since the doctor is putting in a range (110-120), he wants to make sure you know this is systolic.  I doubt it needs to be included.

I am told to type some test files verbatim and I know that means exactly the way the dictator is saying it.  My problem is the doctor is saying ain't and doesn't, which are contractions.  This just do not seem right, right?

Valsalva maneuver.  Lots of docs turn it into a verb, so I would say if it is verbatim, what you have is how to spell it, all lower case.

I had one recently where the doc clearly used the wrong word (something like "she does got") on a verbatim report.  I cringed as I typed it.  Of course, after sending it, I wish I would have either corrected it or just flagged it, since it had to go to QA anyway.  QA corrected it.  That part doesn't bother me, but maybe we need a new definition of the varying degrees of verbatim.  It's getting a bit much.

i would keep adding "The" since pH either needs "the" in front of it or needs to be recasted...and recasting your example, "pH is WNL," sounds like Yoda talking..."within normal limits pH is..." =) verbatim accounts sometimes give grammar grace...plus it has no bearing on patient care...just fixing a lazy doc...my 2 cents...done, am I =)

I rearrange the sentence so that it is not the beginning. If it is a verbatim account you don't want to change anything but type as both letters in caps, PH.

different providers/facilities have varying policies. definitely put the phrase "f off" in quotes, whether you type the f-word out or not. i type verbatim, i.e., if the provider dictates "eff off," then i transcribe "F off." if the f word is dictated in full, then that's what i type. if the provider says "F blank, blank, blank," then it's transcribed "F___," etc., so yes to verbatim. and i always figure if a facility/provider wants it done differently, then they'll let me know!

It depends on the client profile. On my non-verbatim accounts, I leave out the is and are in the vitals signs and for allergies, I just type the Demerol and Penicillin under the heading. That's just how I've been doing it for over 15 years and so far so good :)
