First frozen section came back with positive margins so he went back to excise a second specimen which he submitted S/L "on fos" with the new margin up. This is a verbatim account and this dictator is on 100% QA (all reports) since he is so SPECIAL. I referenced the other reports in his MD data base and found "en Fos" which would have been put there by QA before the report was signed off to the hospital. I can't find this anywhere and was wondering if anybody has a clue?

You must use common sense. The bottom line is to be careful that you never change the intended meaning.

He lay down. He laid down is incorrect. You would only say that if he laid something down, like an object he was carrying. Hope this helps! I was trained as an English teacher many years ago.

Having a little discussion in the office about correct way to use the "followup".  Can someone please clarify... is it one word, should it be hyphenated?  If "the patient is seen today in followup"  or "the patient will return for followup"


Should those two be different?


Another question, does anyone use the word "Esq" or Esquire when transcribing a medical letter to an attorney?  Does anyone even use that anymore?  What is the correct way to address an attorney if the physician is dictating a letter to an attorney?


Thanks for any help.  Looking to try to prove a non medical transcriptionist wrong.

Get the book "Stedman's OB/GYN Words."  I got mine at Barnes & Noble "used and out of print" for $9.00 and it's the newest one out.  Perfect condition when I got it.  It's getting a bit "dogeared" already.  I use it constantly when doing OB/GYN, as I've never done it before.
