Can you hear "1+ swelling?"

For the second, she might have misspeaked. It looks as if she took a stool sample for Hemoccult while doing the rectal exam. I'm thinking she might be saying (or meant to say) "Stool negative for Hemoccult." Then she goes into the urine dip.

Since they start out on the left side,
Subject: Since they start out on the left side,

I don't think this is logical. They start at the rectum, then meet the sigmoid (left colon), next transverse, then right (descending) colon, then cecum last.

But I'd be more comfortable with the term rigid sigmoidoscope than colonoscope. No way can a rigid instrument get all the way around, right?

It is very short. Lead and HCT or HBG are the things assessed for. I would take a second listen to it and see if you hear lead.

I've heard a doc order his new loaded Lexus on his cell phone when he laid the phone down while dictating.  Sounded like he was gettin' a pimpin' ride!  I have also heard love making between a doctor and nurse (they were both married to other people).  Ooohh, aahhhh.  It was years ago, but was so funny.  Sorry, just had to share......

You don't have to capitalize it. It is not meant to start with a capital.

There may be other opinions out there, but since it is supposed to be pH, keep it that way even if it does start the sentence. I like the idea of adding the word THE to the front, though. That works too and solves the problem! Good thinking!!!
