1) Abdominal discomfort, especially over the epigastrium with some radiation to the upper chest.  Differentials include gastric etiology such as reflux disease, gastritis, (s/l wildair, wild bear, wilder) disease and a remote possibility of cardiac etiology.

I really can't find anything close to this one.  Can you help?

Can you hear "1+ swelling?"

For the second, she might have misspeaked. It looks as if she took a stool sample for Hemoccult while doing the rectal exam. I'm thinking she might be saying (or meant to say) "Stool negative for Hemoccult." Then she goes into the urine dip.

I don't think this is logical. They start at the rectum, then meet the sigmoid (left colon), next transverse, then right (descending) colon, then cecum last.

But I'd be more comfortable with the term rigid sigmoidoscope than colonoscope. No way can a rigid instrument get all the way around, right?

It is very short. Lead and HCT or HBG are the things assessed for. I would take a second listen to it and see if you hear lead.

You don't have to capitalize it. It is not meant to start with a capital.

There may be other opinions out there, but since it is supposed to be pH, keep it that way even if it does start the sentence. I like the idea of adding the word THE to the front, though. That works too and solves the problem! Good thinking!!!

You then titrate up to a higher dose. Titration is increasing in increments rather than giving a big dose all at once. Hope this helps.

She stated the account was verbatim, and clients expect to get what they pay for. We can't just change account directions because we feel like it. That would be telling the client, "Sure, I'll do your account verbatim, but only when I feel like it. If I want to change something, I will."

I don't hardly think that would go over well with the client. With a verbatim account, the client expects the finished report to reflect exactly what was dictated -- nothing more, nothing less. If the account wants the MTs to expand abbreviations, they wouldn't ask for a verbatim account.

I thought for sure the first part was "he was still" but my son, hearing me listen to it over and over, actually got that it was "he was told" and then I was able to get the rest.  "He was told about a lupus anticoagulant."  That makes a little more sense than what I had!  Thanks for the help though!  That was a good idea.

The definition of scrumming is.."a part of a rugby game in which the two sets of forwards gather around the ball with heads down and arms linked and try to obtain possession of it." Could they be meaning same as to hem and haw around. His head hung down and arms folded?
