The important thing is not how many staff physicians the hospital has but how many ER visits.  A small rural hospital may have more ER visits than a large hospital.  Some smaller hospitals also have a Fast Track which comes under ER dictation.  I would be surprised if you are the only MT on any account, large or small, as they operate 24/7 and that's how they want their records.
