I don't know what company you work for, but at the one I work for we also see the reports that are done by the facility MTs. What I thought were huger errors by our MTs turned out to be errors by the in-house MTs. Shortly after reporting some of these errors (they were medication errors) we were given more of their workload. But, I have also gone in to reports to look up things and found our own MTs were not editing very well at all - unfortunately it goes on, and what people who do this don't thing about is that they make us all look bad when they do it.
