Electronic Medical Records is just a fancy work for paperless medical records.  It basically means that everything is electronic, not on paper.  Most places use laptops or laptop-like devices and input the information directly into the electronic device rather than dictating a note and it being typed by a transcriptionist.  Some, though very few, still continue to dictate their reports and the Transcriptionist would put what is typed into EMR.
