What are the "rules" for an IC?
I recently started as an IC for a small company after having worked for a large one for YEARS. My question is, what are the rules? If you take a day off, are you required to do that work as catch up the next day? My job asks that I do. Why take a day off if you have to work double the next day and play catch up?
I will write the new "Rules of MT" for free...

I have a Master's in English, work for a major, well-known medical publisher, and I will put out a book that will keep both the dictators and MTs happy by, as the above poster mentioned, following traditional rules of grammar and punctuation...and one that doesn't change just to enable publishing another edition!


How does that sound?

You did not fail!
Not every job is right for every person. It is actually a positive thing that you realize that the company is not a good match for you. If I were in that position, I would give a 2-week notice, as it is the professional thing to do. Good luck! Don't beat yourself up. There is something else good out there for you.

I said, in the beginning.  I have been an MTSO for many years.  I tell the physicians what I charge.  I have lost only 1 account out of many due to cost.  I realize the horrible effect that outsourcing has had, but there are many, many physicians who still care about the quality of their transcription and are willling to pay for it. 
You are setting yourself up to most likely fail...

if you try to start an MT business right out of school.  Unfortunately, people take the college courses for MT just to find out that most employers do require experience.  There are companies that do hire newbies, but one must have attended an "approved" school.  It's not too late for you though.  I see you as having 3 options:


1. You attend an "approved" MT course/start over with schooling.


2. You apply everywhere, check your newspaper, keep looking.


3. You attempt to start up your own business and learn as you go, although I highly don't recommend this route.

...why this is upsetting you so much.  She didn't call your husband and flirt with him or ask him out.  She called looking for your husband's friend, and I am assuming from your post that this woman is a friend of your husband's friend.  A lot of males have female friends and vice-versa.  I fail to see how this is your fight.  I would let it go.  It doesn't sound like it's hardly worth getting yourself upset.  Just my 0.02 cents.

you fail to understand basic economics

the market will pay whatever the market will bear. You say "if all IC's set their pay scale like they should have in the beginning, the MTSO's would have to pay it in order to get their work done and would have to charge their clients enough to cover the costs." What you don't understand is that MTSO's don't set their pay scale either. The physicians decide what they are willing to pay and if you can't bid into that range, you don't get the work. When India came in to the picture, the entire pay scale dropped. Therefore, MTSO's must charge less if they want the work. Therefore IC's must accept less if THEY want the work... and so on down the food chain.

Tell your employer that the money you give to AAMT/ADHI for the 'privilege' of taking their test is funding the progressive offshoring of the American MT profession. If you've been an MT for nearly 30 years, you don't need a CMT after your name. You were a professional MT long before that bunch of con artists dreamed up that scheme to make themselves rich by making MTs think they were on their side. Now we all know better. Don't fund their offshoring scam!

style
I really don't think the guidelines are for the benefit of the hospitals, they seem to benefit the MT company more. By changing the styles daily and having different stayles for different clients, it's nearly impossible to remember, and when we forget something, we lose money, so they win!! English was my second major and I have had QA people change things that are so incredibly wrong that it is totally unbelievable. I have run into 20-year-old QA people who talk like this "Well, that was, like, what we were, like, told to do." Give me a break.
Style

I've been working at home as an employee for over a year - using a style that was provided by the employer - now I have received a message that the style is switching to AAMT styleguide which is different from what I have been doing.  Has this happened to anyone else?  Just wondering why anyone would change midstream.  This is an old account and I don't know if there were complaints or if the style is being dictated by the company or the physicians - who don't really use any kind of style - we just try to make it uniform. 


Style

I've been typing for the same clinic for the past seven years.  Our policy is to make all sentenses as complete as possible to make for better reading.  


I'm going to test tomorrow for a part-time job with another company.  My question is, do I type it like the doctor says it or do i make complete sentenses?


Example:


Doc says "patient has CLL."


I usually would type "The patient has chronic lymphocytic leukemia."


 


 


Try a different style
Those hard plastic earbuds hurt my ears, too. I got some JVC Marshmallows. They have this nice foam on the end that you compress, then place in your ear. It then expands to stay in place and block noise. Very comfy.

Don't eliminate the periods, don't use dangerous abbreviations, and don't use illegal abbreviations such as q.a.m. because they are Latin and English mixed.

Yes put current job. If it is your first
job you will need to show the experience to get another job.  You don't have the give another company any particulars about why you are looking for another position/more work.  Most just want to know when can you start and the hours you can work. 
Nothing will ever be more current than SM
the Monthly Prescribing Reference.  Google it and the link comes up.  It's updated as soon as a drug is FDA approved.
Book of Style

The cardiologists I have worked with over the years preferred Roman Numerals for murmurs, however, all that changed when The Book of Style came out indicating to record murmurs with numbers not Roman Numerals.  I even had a few cardiologists state, put the murmur in Roman Numerals.

The AMA has their own book of style...
and they DO NOT endorse the BOS. 
Book of Style
Sometimes this stuff just gets crazy.  I do not yet have the 2nd Book of Style, but does it really matter that much?  Two months or 2 months -- it does not change the meaning of the report.  I agree with BOS I think it should be spelled out at the beginning of any sentence but really!  Lord knows we type for pennies as it is.  Now we are expected to purchase yet another book and/or CD and retrain our brains.  Some days it just does not seem worth it.  Could any MT with the 2nd Book of Style please tell me if the rule changed for when and when not to hyphen well-nourished, well-developed?  It is currently covered on page 79 of the first book.  I typed it as shown in the BOS 1st one, but got two reports back from QA saying I was wrong?  In my case my report read "The patient is well devolped and well nourished."  The QA person added hyphens?  It is my understaning you use a hyphen when the sentence reads"Well-nourished male/female."  There is only 1 QA person that seems to fault me for this time and time again.  It upsets me because I do not believe I am wrong on this one.  Am I?    I enjoy getting feedback to see how I am doing and learn the occasional blank which I cannot for the life of me figure out but really on this hyphen thing I am referring to the BOS.  It is all there in black and white (good old plain English) page 79.  I am debating if I should fight this or not at my company.  Seems like whenever I say anything -- been down this road before -- this person in QA just gets down right nasty.  I have no problem with QA people when I really did miss something or research something incorrectly.  Again QA can be a great learning tool, but it just seems to me there is always one who is enough to drive the poor MT to tears. 
SOAP is a style.
Subjective, Objective, Assessment, Prognosis. Some offices like to their office notes typed in this kind of format, others don't.
This is actually according to the AMA style guide
AHDI and JCAHO are just following the AMA.
Actually, there is another style guide for MTs (sm)
Medical Transcription Guide: Do's and Don'ts by Marcy O. Diehl ... it differs regarding some style advice so one wouldn't be 100% BOS compliant, but it's a good guide, nonetheless.
You need to modify the style.
Select the Header style, change it to what you want. Do the same for the Footer, then save the template. Close and reopen as a new document and all should be good as gold.
What is the correct style?

My BOS is at home and I'm at work.  If the doctor says 15.6 thousand and then says increased to 18 thousand which way is correct?


15.6 and 18,000?


15.6 thousand and 18,000?


or 15,600 and 18,000?

To get 250 per hour...you really have to have your short cuts in place and be using them frequently, this is what I have to do anyway, and since switching jobs it has been hard to keep up due to the "newness" of the dictators, so my butt better be in that chair typing to make these kinds of results.

FOLLOW UP

When she came home from school , bussing it I would not allow her to go to her room and nap.  She was exhausted from the night before. I told her you are not going to stay on the phone until 1:30 and then sleep and then do the same thing over again. I took the cell phone away for a while and told her she can use the house phone and the phone better not ring after 9 p.m and she is off at 10 p.m.   Kids are tricky though.  I am going to take the portable phone to my bed room at night.   In the middle of the night my other daughter use to take the phone in her room.  Her boyfriend has no rules or curfews and this is part of the problem.  He told her that his mom feels bad that they are divorced so she gives into him.  Until now she respected our wishes.  She is a great , smart and talented kid.  This is the first time I really has disrespect from her.

Follow-up....
I thought I would let you all know I received the keyboard and so far I really like it.  There are so many things it and the mouse can do, that I'll be busy for a long time figuring it all out.  The touch is quiet (which is good with small sleeping children).  It is considered ergonomic, even though is not a split keyboard.  The mouse can be programmed to do all kinds of things (there are like 56 commands you can program it to do if you want).  The receiver for the wireless keyboard and mouse also works as a battery charger.  They send you two sets of rechargeable batteries so one is always ready to go when the others need charging.  Quick and simple to install driver and set up.  I ordered mine off Directron.com and got it for 34.99.  I ordered it on Monday and got it Wednesday and that was with ground delivery by UPS, I was impressed.  If anyone has any questions about it, just ask.  I'm still figuring it out as I go along.
