"Padding" lines is spelling things out, like abbreviations or numbers.  I was taught that when a doc says COPD, when it is the first time in the report, you spell out "chronic obstructive pulmonary disease."  Then, when COPD is stated later in the report, the abbreviation is to be used.  Many account specifics state a report must be typed verbatim, but I still refuse to use abbreviations when they are stated for the first time or in the assessment and plan.... Are you being accused of this or just trying to avoid it??? 10 mm 0-degree telescope
