 I can't seem to find a good reference to this but it sounds like he's saying "triple scope."  Has anyone heard of this or know if I'm hearing right?  Here's the context.  "The patient continues to have no evidence of disease recurrence from her Stage II head and neck cancer treated with chemotherapy and radiation.  A triple scope will be performed."

 "Padding" lines is spelling things out, like abbreviations or numbers.  I was taught that when a doc says COPD, when it is the first time in the report, you spell out "chronic obstructive pulmonary disease."  Then, when COPD is stated later in the report, the abbreviation is to be used.  Many account specifics state a report must be typed verbatim, but I still refuse to use abbreviations when they are stated for the first time or in the assessment and plan....
