I live in Wisconsin and am typing for a Florida Hospital.  He states the patient is from "s/l" Ge-on  ???  Thanks in advance!  It is clear and he says it a few times in the note.  I cannot find it over the internet.

experience is, it lacks credibility especiall when you base it on YOUR experience.  Who are you?  How much experience do you actually have?  What kind of experience do you have?  How many diabetic patients have you encountered in your years of experience?


You can't expect to make a sweeping generalization like "all diabetics have hypertension" and not get called out on it.  Come on!  Know your place.  Arrogance is an unattactive personality trait.

everyone going by their own rules. There is no consistency. And thanks to that we're now pretty much considered non professionals IMO. I say bring on the mandatory licensing/certification to bring the professionalism back to the field!

If not, I would just type as dictated and maybe put quotations around it if you are certain of what he is saying.  HTH

Personal opinion, but I would only makes this a "header" within a report if the dictator state
