This is what I am hearing.  Does this make sense/look correct?  This MD likes to make every word short and sweet.... but I get so confused where to put it all.  Thank you.


 


EXTREMITIES:  There is edema of the right ankle at the site of fracture times two.  Trace to 1+ on the left, significantly diminished pulses equal 0-1 DP and PT lower extremities bilaterally.  Normal upper.

Dr is saying Extremities: No cyanosis, clubbing or edema.  S/L" SETs" or SVTs are in fact in place.

The finger is swollen with a small opening on the s/l fulmer aspect of the ring finger at the DIP joint area.  He is not saying volar or palmar aspect.  It sounds like "fulmer."  I'm stumped.  Any help is appreciated.

Patient has bilateral feet ulcers, poor or nonexistent pulses. Dr. says her skin is thin and atrophic with s/l "zarosis planoy".  I can find "xerosis" which does not seem to apply here.  Any ideas would be appreciated.  TIA.

In view of the ___________ (rep/rub cuff/calf) it was felt that the patient would benefit from full anticoagulation for a short term.

I found this....

Neuroleptic induced Parkinsonian occurs in 15% of patients. Symptoms include muscle stiffness, cogwheel rigidity, shuffling gait and perioral tremor. Treatment is anticholinergics.

Doctor is saying I have noticed a shuffling gait, s/l todalol tremor and some cogwheel rigidity.  It may just be a simple non-medical work, but I can't be sure.
