The patient was previously diagnosed with (S/L theral) meningitis.  First I thought the doctor said "viral," but it really sounds like "theral."

I cannot make out the last word of this sentence. The doctor has an accent and there is a lot of noise in the background. CSF showed a cryptococcal type of cough, 1:512. It sounds like he says cough but that does not make sense. Thanks.

Don't know what diagnoses this patient has, most of the drugs that end in "ivir" are AIDS/HIV drug related medications. If the patient does not have this, that will eliminate a lot of looking.

not sure in this case.  Account goes by BOS, not super nitpicky, but I'm wondering what y'all do.

I think if you would substitute "bilateral"  (mentally)  and say  "both" you might have a better idea how to say it.   Like would it sound better iff you said "both hand"  or "both hands."   To me bilateral hands sounds better.  But again, I don't know if there is actually a rule.  I looked for an answer in "style Guide for Medical Transcription" (Tessier and Pitman) and couldn't find an answer.   


I think if you wrote what the doctor said ....you can't go wrong there either.  Good luck!.

RULE OF THUMB

basic guideline is:
if you have a credible reference, spell it whichever way the reference spells it.
you should never be counted off for it, as long as the reference is credible. if you are, it is a valid challenge to assert.
