Working on hemorrhoidectomy operation and not having much luck with equipment used, namely brand names which I can't seem to find reference for. Thanks.

The anterior approach of Henry was now performed.  Care was taken to preserve all neurovascular structures.  The fracture site was **** s/l rarely *** delineated.  A butterfly fragment was noted.  The fracture was now reduced and the butterfly component was provisionally pinned with small K-wires.

I can hear it very clearly, but I cannot reference it anywhere.  Doc says "Irrigation was done of the wound as well as a bone towel at this time".


Does this make sense to anyone?  Thanks for any help.

Trying to figure out how to type this ---- ortho surgery with Synthes LCP plate.  Says "number two five" Mersilene tape..... Would that be #25?  That doesn't seem right to me and I saw that another MT typed it as "two #5" Mersilene tape.  Not sure if dictator said it wrong in my report-- appreciate any input.

One of my ortho MDs likes using a s/l "Ansbach/Aunsbach" or "Onsbach" bur when doing both cervical diskectomy and fusion and lumbar laminotomy, facetectomy and decompression. I have googled and also searched all of that MD's reports in our database and noticed that the other transcriptionists have left this blank in every snigle reoprt. Anybody ever heard of this?

My question is he is just saying 45 hole, not saying 4.5 hole so do I assume he means 4.5?  I get so confused by this.  This is the sentence leading up to my question. 


A 4.5-mm syndesmotic screw was now inserted using standard AO technique.  I placed the screw in lag mode.  A 4.5-mm drill bit used on the fibula.  A guide was placed in the 45 or 4.5?? hole.

What kind of surgery is it? If he used "lap" laparotomy pads may be the second one. They usually say sponge, needle and instrument counts. Will keep thinking on this one. Type of surgery would help.

FINDINGS:  Epidurogram revealed normal (s/l-cephaloquadet-) contrast flow outlining the right L5 nerve root sleeve in the epidural space with good flow achieved medial to the right L5 pedicle.  The patient did experience some concordant L5 symptoms on the right side during the injection.  Vital signs remained stable throughout.
