My first post and I goofed it!  I just posted this on the Main Board when I saw the Word Board that might be more appropriate.  Thanks in advance!


I'm working on an ortho eval for a patient with a PIP fracture.  Doctor has dictated an unfamiliar term (there's always that ONE WORD) for me.  He dictates, "X-ray examination today shows impaction at the base of the middle phalanx with approximately 70% joint surface involvement."  Here's the word I don't know (spelled phonetically--Sorry):  "A syphiant(??) portion of the joint surface is translated dorsally."



If someone could point me in the right direction for finding this word, I would really appreciate it!

My doc is dictating about a teenage baseball player.  He had an AC joint separation and wants to start the season.  The doc says the patient can "continue to s/l de gauge, continue to do tossing and as soon as he has no pain, throwing and light tossing of the ball"   Sounds like de gauge or dig H (my doc has a spanish accent, not too heavy, though)  Any ideas?  Thanks.

Impression:
1. Left de Quervain tendinitis.
2. Left carpal tunnel syndrome.

Will get EMGs. The patient has evidence of s/l palmarthrorheumatosis. He runs it together, so it may be palm (or palmar) arthrorheumatosis.

Any help is appreciated. :)

There is also a PA view (my understanding is just a different way of standing when taking the x-ray vs an AP view). The way you are typing them is correct.

Would that be #25?  That doesn't seem right to me and I saw that another MT typed it as "two #5" Mersilene tape.  Not sure if dictator said it wrong in my report-- appreciate any input.

One of my ortho MDs likes using a s/l "Ansbach/Aunsbach" or "Onsbach" bur when doing both cervical diskectomy and fusion and lumbar laminotomy, facetectomy and decompression. I have googled and also searched all of that MD's reports in our database and noticed that the other transcriptionists have left this blank in every snigle reoprt. Anybody ever heard of this?

My question is he is just saying 45 hole, not saying 4.5 hole so do I assume he means 4.5?  I get so confused by this.  This is the sentence leading up to my question. 


A 4.5-mm syndesmotic screw was now inserted using standard AO technique.  I placed the screw in lag mode.  A 4.5-mm drill bit used on the fibula.  A guide was placed in the 45 or 4.5?? hole.
