The patient has a  TM perforation.  S/L Olm? process of the malleus has been eroded.  The incus and stapes are absent.
pseudodisc bulge?

I transcribe for radiologists in two states, and lately some of them are dictating  "pseudodisc bulge."   I wouldn't think it would be a pseudodisc, but rather, a pseudobulge of disc.  I've googled both terms, and don't get many hits for either.  What should they be saying?  Is this one of those doctor coined phrases?

States:  A wire was then passed through the needle, the needle was removed, and the triple lumen was placed over the wire and the wire removed. This was sutured in place, all 3 lumens with a positive s/l "flash." 


Is "flash" lingo for blood return? or is he saying something else.  I've not typed these before.
