I agree 100%. I was on a great hospital with MQ but they kept calling me to work in my secondary which is one of the worst hospitals I have ever transcribed, IMHO, over 36 years.  I frankly got tired of the every day call to work in my secondary so I focused on my secondary, mastered the hard doctors and now my secondary is my primary, a hospital which most dont want to transcribe..so usually there is lots of work and bonuses too..guess **I won**, hun?

If you want to have a primary and a secondary, Cornerstone has it. I am the QA supervisor and we are looking for ESL typists for a primary with one secondary. Check out the ad posted in the job section either today or yesterday.
Secondary, tertiary accounts
Have you tried asking for back-up accounts to work on?  I think a lot of us have experienced a slower-than-usual summer this year.  Even though production may be slower/lower on accounts you don't work on frequently, having secondary, tertiary or more accounts to work on helps to get through the droughts.  Good luck.
Diagnosed w/deQuervain's
I've just been diagnosed this week with deQuervain's tenosynovitis.  I've gotten a splint (very awkward!) and have been prescribed anti-inflammatories.  Are there any exercises to do to help w/this?   My doctor mentioned PT, but I wasn't ready to go just yet.  Surely don't want to have to undergo surgery--thanks very much for any information or personal experiences!
Training on secondary accounts. Can you refuse?
I have a great primary account. I realize I need a secondary for slow times but the secondary account blows big time. It is laborious (many fields, special requirements that are tedious, etc.). Can I say, "No, but thanks." I reach my required line count with what I have on the primary account even though I am not working a full eight hours. I have taken on a PT job and just fill in empty space by turning around and typing on the other computer when it gets slow.

Any feedback or advice would be appreciated. Thanks!

It is 9 in the morning and my main is out and secondary going out from Amherst. No wonder everyone

is leaving. How can you be paid on incentive with the new pay plan and be out of work and stuck in accounts you never heard of. That has to be the most ignorant stupid thing I have ever heard of. Totally ignorant.


Familiar with primary/secondary insurance?sm

My son is currently covered under my husband's (his stepfather) policy.  His father and I have talked about changing him to his insurance come open enrollment time later this year because his coverage is now much better than ours.  I am considering actually leaving him on my husband's insurance too though, in the hopes that primary/secondary coverage will leave less out of pocket expenses...because it costs us no more or less with him on our policy anyway (same family rate regardless of the number of children covered). 


My question is this.....if our policy has a huge deductible then pays at 80/20...and if he was also covered by his father's policy that utilizes office copays...how would that pay?  Would his father's policy still not kick in and pay anything until we met the deductible on the primary policy?  If so that's pointless...but I don't see the secondary insurance company wanting to pick up all the expenses that the primary won't.  Anyone familiar with this situation that has any suggestions?  Would be greatly appreciated...very confused.  LOL  Thanks!


Have you been diagnosed with occipital neuralgia?
I have been diagnosed with every headache there is. I have had 7 surgeries this last 1 1/2 years plus am on multiple medications. I even went so far as to have a C2 neurectomy, which was unsuccessful. My next surgery is hopefully my last. I have an occipital stimulator placed, which works wonders for the headaches and pain in the back of my head. I have to have it hooked up agan, as they removed the wires due to an 8-month long postop infection when I had the wire revised. For frontal ones, I get Botox injections every few months. Not only am I a sufferer but I transcribe for a headache and pain clinic. Feel free to post to me.

You can get rebound headaches, especially if you are using Excedrin Migraine or more than two Fioricet a day. For other people like me, it is a combination of things that are causing them.

Just diagnosed with type 2 diabetes...sm
I would really love to hear from others who have diabetes and any tips on how to live with it.  I just got my testing supplies, a diet guidelines, but now have no idea what to do from here.  Of course my health insurance as of tonight at midnight ends for two months, as I had to switch companies, so hope that does not cause any problems, but we will have to see.  Please feel free to email me or post here.  Thanks. 
Use main pattern, secondary backup patterns when first SM
isn't enough, and then deviate by choosing the "intuitive" choice for an abbreviation that pops into my brain, on the assumption that it'll be the one that pops in when I need the expansion. Usually it is.

I've always had the drop-down list turned off on my ShortHand program, so I can't speak to working that way. A lot of people clearly like it a lot.

However, when editing rather than transcribing, I question how well one can read ahead for corrections to pop down to and scan the dropdown list too. Fast-reading ahead and even making corrections while listening to the squeaky speeded-up dictation as it trails is how to make good money editing. One needs expansions a lot less when editing, of course, but they're still extremely handy. So if you choose to keep developing a system to work directly from as much as possible, rather than coming to depend on scanning a list, I think it should pay off nicely over the long term.

OTOH, since editing does need a lot fewer expansions overall, an Editor would come to memorize all the usual ones anyway and only have to stop and look at the list now and then, never in some documents, often in others. I'm not tempted to change my routine, tho.

having trouble adjusting and trouble finding a doctor who understands. If others with fibromyalgia wish to correspond, I will post my email address. There are no fibromyalgia support groups in my area and corresponding with someone who understands would be great!
Coffee is a stimulant. My oldest was diagnosed with

severe ADHD.  Meds didn't work and made his heart race to the point that he was very uncomfortable.   He is now 16 and learned coping skills, but he has been off meds since he was 8.   I always tells parents of newly diagnosed children to try giving their child coffee or Mountain Dew on a weekend when they have nothing else going on.  Sometimes it works, sometimes it makes the kids bouncing balls, which is why you don't want to have any other plans that day.  


The drugs are so bad if you can come up with an alternative it would be so much better.  The nurse at our doctor's office told me to take a pill one time to see what my child was experiencing and it was not pleasant.


 


 


 


In time past my child was diagnosed
with a disease that is catastrophic. He was in college and quit that day. I was employed in hospital setting and immediately sat up Cobra, and carried it for about 4 months I think until which time he got a job and got his own medical insurance. My insurance covered as long as the child was in college. I knew with the disease if no insurance bridge would never be able to be covered again and with the cost of the medications needed just an overwhelming thought and even though I have money, would never be able to cover the costs of thousands every month to be paid out.
Has anyone noticed lately that there has been a lot of people diagnosed with breast cancer?

I'm wondering how you came to the conclusion that she has manic depression. Was she diagnosed

with this?  From what I read, it doesn't sound like manic depression to me.  Sure, she has some issues going on there, major depression would be a pretty good guess, but does she have any manic episodes?


My ex-husband and ex-mother-in-law were both bipolar which is the new and improved way to say manic depressive and they were the exact opposite of hypochondria.  During the manic phase, they felt like they ruled the world.  According to them, they had never felt better, thought more clearly, or got more done, but the exact opposite was true.  They never slept.  They never finished anything they started.  They were incoherent at times, basically jumping from one subject to the next so you could not carry on any kind of coherent conversation with them.  Sometimes they were even delusional - my mother-in-law thought she was in love with a televangelist and divorced her husband and moved to be closer to the televangelist.  My ex-husband thought he was the victim racism when he got a speeding ticket.  He claimed the officer had something against Hispanics.  Nevermind that my husband was maybe one-eight Mexican on his mother's side and has a German last name.


Those are just a couple of examples of manic behavior.  It can even be more bizarre.  I could write a book based on what I've seen first hand and it can be very scary.


IF it is secondary income, where is the harm? As long as you work around your kids (at night, naps)
a big plus when you are around kids all day and bring in a little cash to help out the household.

Cold fingers often - diagnosed with Raynaud syndrome many years ago.
I usually try warm water first - if that doesn't help, I have these little packets, normally found in the camping section of stores called hand warmers. One is called Hothands-2(up to 10 hrs of heat), another one called MEGA warmer(12+ hours). One you shake to activate, the other just expose to air - then place them in a pocket or glove. I wear sweats with pockets - put them in my pockets and warm up my hands.

Found these several years ago as we go camping; however, hubby loves going out in late Sept & I'll swear would go all winter if he could. When he's comfortable, I'm turning blue!! He found these for me - and, they also have foot warmers (6 hrs heat), which sometimes I will use in the winter if I'm wearing my moccasins - with socks - while transcribing!!

Please, I desperately need some motivational tips...I have been in a funk for quite a while now and can't seem to get over it. Plus, I am in the middle of moving--yuck--and trying to pack, clean, cook, work, etc...AHHH, the list goes on and on and on... Thanks for "listening", just had to vent...but your tips are more than welcome and appreciated!!! this is what's gonna happen to me...any second now!!!

That is my goal. It is really a simple, obvious solution but man is it hard to actually do it--I have yet to achieve this. Thanks for letting me know that it has actually been done by a for-real MT! So easy to let it take over your whole day/week/life. And it is never enough. I want to be like you when I grow up. :-)

my $$$ and I do make a decent salary, made even more in the past when I had more than 1 job but cut back on my own. I could care less about a pat on the back- higher ups are not there to give huggies - my goodness, do your job, do it well and enjoy what you can buy with the money you make or get a third job if 2 jobs are not enough.....

This sounds crazy to me. In the time it takes you to push a button, reload your change ect., you could be typing more lines. But if this works for you great! To me it sounds like a waste of time.

I had nothing to do with being outsourced to VR, not my choice but it came. I have worked in this business since the 70s- outsourced twice and I care nothing about pounding the pavement in my late 50s to look for another job- I stayed with the company outsourced to. I love VR (some straight) and can hit over 600 lines an hour on VR which adds up to a good salary for me and what I want and need. VR is not going to take the place of editors- the system 1 month will start making same mistake over and over that has not been there say 4 months ago so a human still has to listen and change. I get occasional straight typing. Why, oh why should I refuse to work for a company so you can be satisfied or happy? I owe you nor any other MT I know anything. I work to satisfy myself and my family. I have the job I love and hope it continues on. By the way MTs have been fussing for some time about the possibility of things like this. I would suggest younger MTs if they think this is the case- find SOMETHING else to do. My golly. Why would a person sit around on the Titanic when you think there might be an end you might not like?? I myself very satisfied. Thank you!

I'll take some of that motivation, too.
My neighbors were fighting all night after I stayed up too late discussing our home remodel with DH.  I can't bring myself to get busy this morning.  I want to go back to bed after only two hours of sleep in a row last night.  I'm also excited because if we do an addition on the house, I get my own office with a closing, locking door.  I'd rather be drawing up floorplans and taking a nice long bath than working.
I used to go to the gym, but lost motivation - so now
I have a treadmill and an exercise bike in the house and I'll jump on them for 10 minutes at a time throughout the day - it's a good break from work and the time adds up. I only do 30-40 minutes 5 times a week, but it has helped me stay in shape! I also lift 5 lb. weights a few times a week - nothing strenuous, just a few arm exercises my brother, who is a personal trainer, taught me. Good luck - I know it's hard when that fridge is there all day long. I tend to eat when I'm stressed so it is particularly difficult on certain days.

ok I feel sooo unmotivated to work even when I am right in front of my computer. I am just off task, thinking about other things. I lack the focus and brain energy to type. When I drink strong coffee or energy drinks, I am super productive. I enjoy typing and get lots done when I have been using caffeine. What is going on here?

Do i totally understand! I have been doing this for 10+ years and keeping the production up lately has been a real struggle, not only financially but emotionally. Hence, I am returning to school. It is giving me something else to focus on - a goal. Otherwise, I feel like I will be typing the rest of my life. Take some time off??? I would love that !!
1500, lucky if I make that, lost my motivation, how do you guys do it ?

Never get the same dictators, all the OPs are not  the usual appendectomies, T&As, etc, way more extensive, neurosurgery, spinal, etc.  Have 2 accounts, over 150 pages of account specifics, 2 systems in and out, type about 1-2 report from backup account, just enough to waste more time closing out of ChartScript and loging into C quence, then back to CS.  Typed about 1 H&P this  year.  I have no normals.  Rarely ever get ESL dicators though.  Giving myself about another month or two.   Lost all motivation.  How do you guys do it ?   I really need a pep talk.  I just can't concentrate any more.   


Generally 200 lph, but depends on work type, I can do more with OP notes, has to do with motivation

I've just had the worst and the most unproductive working day of my life.  After several days of not a heck of a lot of work, I sat down today hoping to make up some lines.  So my internet wigs out on me.  Cable guy comes, fixes problem, I'm back in business.  About 3 ridiculous family emergencies later (loads of sarcasm here), my phone goes on the charger and I ignore the constant ringing for 2 hours.  Since I work at home, everyone thinks I've got loads of time to chat. 


My SIL was kind enough to take my boys to the pool today.  I was so happy to know that I would have 4 hours of peace and quiet to be able to transcribe.  But since my internet was not working from right after they left until the moment they returned, that didn't happen.  My son comes home from the pool with bleeding and raw feet (Athlete's foot) that I have had no success at treating with the Tinactin the doctor told me to use.  So, I A&D his feet, put some socks on, back to work.  Then my youngest dumps a box of Cheezit crackers on the floor and decides he will walk on them until they are very finely crushed.  After an hour in the kitchen, I'm back to work.  Not for long though.  Older son decides he wants a bowl of coco krispies.  They also end up on the floor, scattered from one end to the other.  Then big brother decides to use his younger brother as his sketch board.  So I spend about 20 minutes trying to get pen off my little one's face. 


Please somebody, tell me I am not the only one with these problems.  I'm beginning to think the only time I will be able to work and actually accomplish something this summer will be if I work when my little monsters are sleeping, which means no sleep for me.

Have you actually done an "Edit" when your cursor is in the patient name field on doc info screen? I'm not logged in right now, but I think you actually need to edit the patient info ... Of course, on some accounts we are not allowed to do this nor do we have privilege.

this morning and my count was 690. That was with excellent dictators and hardly having to stop. Being told 1000 lph, in my opinion, is totally unrealistic.

cost that much money? Most places of that nature don't charge a fee, but rather are made up of volunteers & activists who work together to try to bring about change.

That was exactly what one doctor said to me when he called wanting my services and I told him what my charge was. He was in a town 15-20 minutes away and since tapes were the "in" thing", I charged for picking up and delivery, minimal charge.

Well, he thought that was ridiculous and told me I didn't have any "expenses" associated with this job, so what was I charging him for other than doing the reports. BTW, he got my name and number from his partner in the office who thought my charges were more than reasonable and willing to pay for it to get what he called "great" service.

Of course, we have no expenses, we live free, not having to eat, buy gas, pay all the same bills they do. You know, the necessities of life!!!!!!

Lisa -- this is what I was writing about before. What you are describing sounds like a trigger point that needs to get resolved. The Chinese call trigger points "blocked energy channels." It could be trapped proteins or by-products of cellular metabolism or scar tissue (adhesion). A well-trained Rolfer or trigger point massage therapist could help here. That's what I meant by not just doing chiropractic alone without soft tissue/body work. The old saying, the knee bone is connected to the hip bone -- by way of muscles, tendons and ligaments. All need attention before it's too late like joint damage! If you're getting numbness, chances are you may have nerve impingement (no diagnosis here). Stretching without working out the trigger point just adds to the muscle flexion or extension problem that tightens around the nerves. If the muscles are out of proper position, so will the nerves in that area. A good resource for you would be a trigger point manual with locations of muscles (insertions points) so you can learn. My recommendation would be to see a qualified Rolfer/trigger point massage therapist to teach you how to help yourself.
