I have that problem once in a while

and just like you, the patient always seems to be in the middle of the file. What aggravates me is the doctor will swear he dictated it because "I put a check mark on it". Yet, it was not dictated, but he will then dictate the patient and I must put "Re-Dictation" on it. Now, THAT aggravates me.


I believe the secretaries in the office just throw out the patient dictation if they can't find that name on the day of visit.  I may spell the patient name wrong (they don't send me a list of patient's and the doctor doesn't always spell the name out if it is a difficult name), so I type it as a sound-alike. When I asked for a patient list a few years ago, they said they are not computerized. They are handwritten in the appointment book and that's all they have to go by.


When I DO find one that was sent prior to requesting it again, I charge them for it again. After all, sometimes I have to look through a month's worth of files for one 6 line note. That works for me. They don't request files that often any more.
