 My way is to do the best I possibly can and send as little as possible to QA.  I'm old school. I really think there are few people much better than I am at my job, ha-ha.  Not really, but deeply resent some school teacher-type coaching me through my job and making smarmy comments about needing a course in cardiology terminology for one dumb blooper I made in the face of an extremely good record.  Fill them blanks the best you can and make them hunt out your mistakes, I say, make them look for them.  God knows they'll find them anyway.  Give me the good old days when if the doc was a jerk, the medical records chief would sit him down and tell him to straighten up his sorry dictation or else and yes, the doc would shape up, at least for awhile.  And to give the old docs their due, they expected us (and thanked us) to straighten up their dictation.  Sigh, the good old days.  That common sense world is gone forever.
