That is why I started my own business. It really depends on who you work for and if you get a great account your lucky. I unfortunately wasn't that lucky and after 15 years a GF and I do it locally for doctors. Much less stressful. Glad you can do that!!

I'll bet I work for the same national! While I have great dictators, seriously they never say the
same thinge twice, save one surgeon who always ends his op reports with the same paragraph, which, of course, I have in my expander. I mean it - there are probably 30 doctors, and I never ever get a repeat or any pattern at all! Again, I have a huge expander that I've set up on my own over the years, and rarely type more than a few words "longhand", but I still am not hitting over 200-250 lines an hour. Oh well! At least I have great dictators, right?

Could maybe see clinics going over to
full EMR with doctors documenting, but it would be very hard in hospitals for physicians to do this considering how complicated some dictations are.
Try hospitals and/or clinics-
I don't know about other places but in at least 3 of the major hospitals in my area they have people work both in house and at home. Everyone works in house to begin with and has to prove themselves before going home. Some people (like me) just prefer working in house, so it works out.
Local Clinics
I would seriously try to find work at a local clinic if you're finding it hard to get hired with a National. I also went to a local community college. I didn't take me long to find a very small local MTSO to hire me, thank God they decided to take a chance on me. I now work for a local nephrology clinic. I definitely know though that if I tried to go out on my own, there would be no way I could have done it. Good luck with whatever you decide to do.

Or if you have a small local MTSO, they may be willing to give you a chance. That's how I started. I trained with the MTSO at her office for a couple of weeks and then I started working from home for a local nephrology group and I've been there for the past five years. Try not to get discouraged, although I know that's a hard thing to do.

what you have just described is the best and most cost-effective way to get quality transcription. At the hospital where I used to work, MTs are treated like the scum of the earth. The nationals want us, but pay peanuts. By eliminating the middleman and the overhead of having enough office space for MTs, a hospital/clinic can pay the MTs what they're worth, the MTs can afford to keep working for them and stay on for many years, gaining a better knowledge of the doctors and the institution. Both that and having decent pay and a more flexible schedule means happy MTs, and happy MTs will go the extra mile to produce top-level work. It sounds like your employer is ahead-of-the-curve in the way it deals with its transcription needs.
I think the MDs that care in their offices or clinics sm
keep their trans in-house. My personal MD does and overseas every last little detail and she is tough! The ones who dont want to mess with it go to a service and let the service take responsibility. So neither will care....IMHO

that still have in-house. My local hospital did this and the education/experience/hands-on help provided was golden. Then go home and work - you will be viewed as a step up if expierience is in-house.

jumping on the save lots of money and offshore bandwagon! We just can't win. It's over everyone :(
I am an IC and my accounts are clinics and doctor's offices
So I have weekends off unless the docs dictate late on Friday, in which case I have the choice of working late on Friday or getting the work done over the weekend.
With clinic work, you don't have the constant flow that you have with a hospital. You can't work harder or longer hours to make more money. There is a specific amount of work, and when it's done, that's it. Your income will fluctuate. But if you're willing to put up with that - you can have your weekends off.
ALL of the clinics I work on use SS #s as medical record numbers, and many of them cc to the
patient at their home address, which is conveniently typed at the bottom of the report. Seems pretty straight-forward to us, then why can't the govt figure this out and put a stop to it?
Is this Holiday a major one? Clinics and offices closed?
I guess I am out of it. How big of a holiday is this? All the kids home from school today? Post offices and banks closed? Seems rather quiet work-wise. Wondering if I should start cleaning the house or keep staring at the no jobs to do in my queue.

Yes, I always have the notes. One time, it was an entire day worth of clinics except for one!
How did they just get one note and not the rest? There have been a couple of times when the doctor just didn't dictate on that particular patient, but most of the time I typed the notes, printed and delivered the notes, and then they say they never go them! And I always have them on my PC and backed up on CD.

That's what I did. I first started out working for a very small MTSO and worked at their office for a couple of weeks and then they moved me home and I started working on a single speciality account. I'm still working for the office after 5 years (altough the MTSO closed her business).
Any hospitals/clinics hiring home-based employees on an hourly basis? TIA

I don't know about "natural" approaches for the house. I have always used flea bombs in the past before the miracle of Advantage and Frontline changed my life. Have not had a flea problem at all since I began using those products about 10 years ago, dose all the dogs religiously once a month with the Frontline Plus except in Dec.-Feb. to save a few bucks and because the ground is generally frozen by then and no fleas are scurring about. -- Hope you get flea free soon.

Legit place...hmmmm. There are a few out there. Whether they have clinic work is another story. I'm sure someone will have suggestions. But, isn't it cool that you'll probably already know the doctors no matter where you go!

Vonage about 6 months now and I love it. The only mistake I made was trying to keep my original phone # - I wouldn't recommend that. You have a temp # for a while, and the other provider can fight for the old #, etc. Go to vonage. com. They have new features all the time! There is even a new soft phone that is a phone on your screen. You don't need a real phone when working - just headset. It has 800 #s you can have very inexpensively - great voice mail. You get emails when you have new messages, etc. I only wish I had used them a year ago! Nothing but good things to say, and I complain about everything!

You should feel great as a transcriptionist if they think your work is that great!!!
It is not your fault they approached you. I think it would be a different storey if you were the one searching for the job to take away from a company. They obviously want to get rid of the company and if you didn't take the job they would probably ask someone else. You should enjoy the better position and not feel guilty!!

if your source says they cherry pick and only do American dictators? Sounds like never. But you knew that.
PS: Are you absolutely sure your source has your best interest in mind with this detailed ratting on another person regarding Indian cherry picking doing non-Indian work?

I can take an ESL more than I can take an American who speaks fast just to show off to his buddies or because he has a hot date and is dictating from his car or worse yet, if you can't understand him, he has an excuse why his report is incorrect, he can blame you for it. They do it, believe me, and they are the ones who should be shot down. Give me an ESL any day who says thank you very much and is trying hard to learn the language. Some of these fast-talking big shots could care less about us. I had one guy who could fit 15 discharge summaries on one side of a 15 minute tape and everyone would send him to me because I was the outside service. When he was spoken to, he just threw his head back and laughed. Ignorant!

don't you just love it when you have to turn up the volume to hear what the doctor is saying and then in the middle of the dictation he finally puts his mouth to the mic/phone, blasting your ears; so you turn it down then by the end of the report you have to turn it back up.......no wonder I have ringing in my ears sometime 
Bad dictators
Chewing gum very loudly, blowing bubbles while dictating, drinking something and then chewing on the ice....all in the average day lately. Don't these people stop to think that someone has to listen to this? That they are chomping incessantly into someone's ears. No common sense. Maybe once, someone in authority should remind them that these are legal documents and need to be transcribed as accurately as possible, so no chewing, drinking, talking while yawning, etc.

I have got to one-up y'all... there is a woman doctor in our group who actually takes her dictaphone INTO THE BATHROOM with her and, yes, that's right, uses the potty right while she's dictating...
ESL dictators
Maybe the ESL docs, knowing that the Transcriptionist must be having a difficult time with the language barrier, should hire an English speaking person to do their dictations. It would free up their time for being a doctor and seeing patient's, give a more accurate transcription, and a better quality report. Wha'cha think???

Seems many MTs have this complaint. We should be told the correct percentage of ESL dictators BEFORE accepting a job. Imagine all the money a company would save by going over details with each new applicant honestly and thoroughly in the beginning instead of making the job sound good and then have a disappointed new employee, who will quit and the company will have to advertise and hire again. Advertising and training on a computer system is costly, including the time involved etc. The more a company can do to retain its employees, the more money it will save in the long run. Also, pay based on production, not quality of work, is counter productive. Imagine if each Transcriptionist was paid a salary and did quality work and no QA people were needed, except to pull random reports of new employees during the first 30-60 days or so. I feel there is too much QA and not enough emphasis put on quality reports from the beginning. Many transcriptionists feel, the QA people will fill in my blanks, yet they may be able to find the word/words themselves if production was not such a huge factor. But too many employees are trying to meet a line count. With a salary, you can establish a budget and know what to expect in your paycheck. I've transcribed for 25 years. I love my work. Since outsourcing and pay by production have been instituted, I feel pride in transcribing has suffered. Give an employee a job they can feel great about, benefits, and treat them with honesty, fairness, and respect and they will be glad to work for that kind of company. Hopefully, there are companies out there like this It's up to us to find the good companies.
Of note, 10 years ago transcriptionists were being paid around 10-11 cpl and $3.50-$4.00 a page. What happened, outsourcing!! We used to get a higher salary than coders, but not now. We need to keep our work in the USA.

I had my share of bad dictators this afternoon too! Is it full moon? I am sure I had your doctor's wife. LOL Can we band together and chart a bus to deliver them to the firing squad?? LOL At least maybe would could let the air out of the tires on their golf carts! Do these people listen to how bad they are???????

You ever wonder that when the powers that be become aware that you are making lines on low volume work days, they somehow sneak all the worse dictators your way? I have had bad dictators, none of them in sequential order, and am no where close to my lph this morning.
