Subject: Is this a pediatric patient? In Massachusetts, maybe?

I found info regarding special education assessments.

http://209.85.165.104/search?q=cache:XsZKFWIq3yUJ:www.doe.mass.edu/bsea/decisions/07-1194.doc+seizure+disorder+CMR+card&hl=en&ct=clnk&cd=1&gl=us&client=firefox-a

Googling "603 CMR §28.04(5)" from the first link gives more info.

Long shot, but what the heck.
