Doctor states - "echocardiogram showed prominent eustachian valve as well as (s/l) false cord in the left ventricle without compromising the left ventricular outflow tract."    Any help appreciated, thank you.

Pediatric anomalies

This has been a tricky one for me.  Newborn female, Vater or VACTERL anomaly.  In the physical exam the doctor is saying "imperforate anus, only single opening at the" what sounds like cranium  "which is the vaginal opening"  I have found a lot of web sites, but nothing that says the cranium as being an opening.  I'm stumped!

For the migraine headaches, the patient is going to continue on the Elavil 50 mg. However she was concerned about breast feeding. The information available does show that it does cross into the breast milk, however in very small doses. Pediatric calcification is that there is potential toxicity.


Last sentence is what ASR spit out, problem is that is what it sounds like, and I'm flummoxed!!!! Help!

In the history, the doc dictates "We initially saw this young man with an episode of **sid-nims-corrhea** in 2003. There is nothing else to help me figure this out, and I have tried multiple spellings. He is a very clear doctor most of the time, but I am totally stumped. Any guidance would be greatly appreciated!

Doc is saying: EKG shows: It shows him to be in sinus rhythm.  It shows an old anterolateral myocardial infarction.  It also shows a rightward axis, a right bundle branch block with  SL/"Bog cicular" block. I see a Fascicular Block, but I keep hearing Bog.

Cardiac cath - he stutters, so not sure if he is saying -gray gradient- across the aortic valve or just gradient, because I can't find gray gradient.  Also s/l he is saying -peer- pressure was found to be 22/10 with a wedge of 12.  Peer?
