Unfortunately, it's a one-word sentence: "HEENT: (s/l Bilcolocephalic.) His conjunctivae are without erythema or mattering. He seems to visually track to light and person well. He has a symmetric bilateral red reflex."

if it helps any- patient has past history of renal insufficiency, nephrocalcinosis, retinopathy, and nectrotizing enterocolitis. I really am stumped, any help would be great.

Pediatric anomalies

This has been a tricky one for me.  Newborn female, Vater or VACTERL anomaly.  In the physical exam the doctor is saying "imperforate anus, only single opening at the" what sounds like cranium  "which is the vaginal opening"  I have found a lot of web sites, but nothing that says the cranium as being an opening.  I'm stumped!

For the migraine headaches, the patient is going to continue on the Elavil 50 mg. However she was concerned about breast feeding. The information available does show that it does cross into the breast milk, however in very small doses. Pediatric calcification is that there is potential toxicity.


Last sentence is what ASR spit out, problem is that is what it sounds like, and I'm flummoxed!!!! Help!

It is EVERT, not AVERT, right? I have always typed it with an E but I have a PA who keeps insisting it is A.  I always thought Avert applied to gaze, as if to look away.  Just want to make sure everting the eyelid is the right term.  TIA.
