at times over the years when I had large group practices. It is important to stress that these were accounts that I knew inside and out. I could type these docs in my sleep. Same specialty. Could not use many macros however because each dictation was different (occupational injuries). I did average 3,000-4,000 lines per day, which took me about 8 hours. Trust me, by the end of that day with only two quick breaks, my fingers, wrists, and neck took a beating. I no longer have my own clients (burned out with operating my own service over the years). I now work for a national and get a wide array of specialties, which varies my work drastically on a daily basis. Once you have to fool with different account specifics, specialty templates (neurology wants it this way, endocrinology wants it this way, etc), there is no possible way you can have that high of a line count unless you work constantly, have no life, and do not sleep! Or unless you have macros or use short cut software to produce greater than 50% of each report. Kudos to those who claim that high of a line count. Trust me, your body will pay for it!
