We do have our QA fill in names & addresses
All of our clients provide us names and addresses of referring physicians. Our QA maintains the lists, and we only ask that our MTs spell them phonetically. QA then corrects any mistakes.

As far as patient names, some doctors spell them for us and some do not. For those that don't, they get their reports back with their patient names spelled phonetically, and it's up to them to correct any mistakes.
As soon as everyone posts their Names, Addresses, & Phone Numbers

Changing Excel 2003 Addresses Into Word 2003 Addresses

Hello !


I have a large amount of addresses listed on an Excel 2003 spreadsheet.  I need these addresses for Word 2003 letters.  I end up copying the address from Excel and pasting it into Word, but as you can imagine, the pasted address is in a straight line and I end up having to type the address so it displays correctly for a letter.  Does anyone know if there is a simple way to change the Excel address format to a Word letter address format?  Thank you for your help.

Resident....
I have worked in/around hospitals for over 40 years and your situation is different. Most places consider any MS of any year a student - that is why they are called Medical Student. They are in school, have not received a degree. Most go for 4 years. In the 3rd year, they start doing internships to get them ready for Residency, but they are still called MS3 and MS4.

After they get their MD, then most go on to Residencies and then they are called doctor.
I had a resident the other day who kept saying

P-tosis.  He didn't sound much older than my 20 YO DS. 


I agree with the cadiac docs though.  I think they are some of the worst.

I think you guys are confusing a 4th year medical student with a 4th year resident - only the resident is a doctor.

Myrtle Beach and the surrounding areas have to be the most wonderful areas to live ever...unfortunately two years ago I had to leave the area due to my hubby's company transferring us. When you get to go to Myrtle savor every minute of my "home" for me. Its wonderful...take in the fresh salt air and know you are in a little part of heaven.
Okay, so I've got this resident

on the line dictating and I'm actually thinking to myself - wow if he's this bad now, what's he going to be like when he's the doc and has no one telling him how to do it. 


He's trying to dictate in that choppy style   (cutting out the words that help to make things understandable).  I'm guessing he thinks it's saving someone money. 


He wouldn't be half-bad to understand, if he would just open his mouth a little bit wider when he speaks.  It's like he's purposely trying to see if he can "throw his voice".  Maybe he secretly wants to be a ventriloquist. 


Seriously, I think the part of the resident's rotation should be in the transcription department.  If they spent even 1 day doing nothing but transcribing, maybe they'd try to be a little bit clearer when they're dictating. 


Okay - done complaining - back to work! 


I have a resident this morning
who is literally spelling everything under the sun.  All medications, the word "supple", etc.  He is driving me absolutely mad.
Resident dictates at end of report...

"Note to transcriptionist...thank you for your time, thank you for your service, and have a great day."


 


WOW, that doesn't happen often, and when it does, it sure feels nice to know we are considered colleagues and not just a typewriter on the other end of the telephone line!

...lemme tell ya, after hearing that a couple of dozen times it is SO tempting to just give it to her (perhaps in quotes, but give it just the same), but so far I have resisted temptation.

As a resident of Louisiana, I ask that you all pray not only for those poor souls in New Orleans but for the whole state and surrounding areas.  The aftermath effect is going to be widespread. PRAY!   We are already experiencing food supply shortages.  Woman fighting over a loaf of bread in a Wal-Mart. PRAY!  Our kids may not be able to start school because there is no diesel or gas for the buses.  PRAY!  For this I urge all you have enormous SUVs just to ride around in to get rid of them.  The worst is yet to come.  PRAY!  There are reports of the centers having out breaks of riots not only in New Orleans but in the other areas housing the displaced people.  PRAY!  Also, help any way you can.  If you can not afford monetary donations, please give your time at one of the shelters. 


All our lives have and/or will change from this tragedy no matter where you live.

Had a resident who practically giggled through his entire dictation....
because his psych patient kept asking him if he wanted to see her underwear (she was in her 70s). I didn't find the woman's comment so funny, but this doc had the weirdest laugh...it was like Ernie from Bert & Ernie on Sesame Street. Weird.
As far as addresses...
Depending on the state you can get a medical address book, which is a lot easier. Lists all docs, hospitals, nursing homes, etc. in alph. order.

I could do better (only 3 years exp. here) if I did not have to look up addresses - sm
I have good days where I average 200 lph sure, but a lot of the accounts I do require me to look up the doctor, so I have to go to AMA, or the hospitals directory on-line, etc. That really eats time. I limit it to 5 minutes generally then move on. I am also guilty of not being totally focused so that does not help as well. My goal this year (once the kids are back in school, 12 more days) is to buckle down and get serious. My goal is $100 a day which I know I can do and still have a fair amount of free time (afternoons off basically). But is it nice to hear of those who do well. Thanks.

I usually use MSWord and tables, and sort by fields, but you can do the same in Excel also.  Hope this helps.

You can't tell what sex the person is, and it just sounds so pretentious and stupid.

I have to be careful with addresses, get "vagina" when I put in "VA" - sm
have to keep on my toes....even if I delete it smartype will put in some other va....word, so I just try to be extra careful when doing addresses. Kind of funny though.
The e-mail addresses are invalid a lot of the time and
they don't usually use the same e-mail address twice.    They put all that jibberish in there so you can't pick out a word or phrase to put in your filter that will block them out.

HIPAA only addresses PHI (protected health information) would is defined as anything that SM

specifically identifies the patient, i.e. name, MRN, SSN, account numbers, etc.  As long as all identifying information is removed from a report, it poses no HIPAA violation to keep it on file.  Furthermore, as an employee of the hospital in the medical transcription department and charged with peforming QA, I have legitimate reasons to access a patient's chart.  HIPAA says so.


Go back and read your HIPAA laws again.


Anyone know how I can take addresses from a Word document and transfer them to mailing labels?

I have several hundred typed out, but cannot for the life of me figure out how to turn it into a mailing list.  I have downloaded Avrey Wizard, but when I try to do it, it asks me about field delimiters and no matter what I pick it doesn't work.  Maybe there is a simpler way??

re: looking up names
If the doctors do not bother to spell the patient names and the offices do not bother to provide me with a patient list for the visit date, they get a verbatim spelling of the name and a note to please check the spelling of all names. I don't have the time to dig around on the internet looking for their patients names and I do not feel it is my job and I do not expect my subs to do it. If they provide us with a list, yes, I expect them to use the list but otherwise they get what they say for names and the office has to verify the names. If the company you work for pings you for QA issues by doing this, then I honestly would find someone else to work for.

I am called Grammy by my grandchilden and "Mom" by my sons. However, my daughters-in-law were told they could call me by name as they have their own Moms. One does and the other one doesn't call me anything. I always sign cards, etc., "Mom and Dad and then our last name, so they will still be part of us, but these girls obviously are not as close to me as their own moms. If they did call me Mom it would be okay, but I don't think they think of me in this way, I would be honored if they did. It's difficult. Most of my son's older friends call me by Mrs. and a few, very few by our first names and it's okay. I don't like being too formal. You'll find with grandchildren at some point they will try to use the first name and we always snicker at it,it's just something each of them tried very, very briefly, perhaps once or twice; don't make a lot of it and it's a phase, they get over it quickly if you take it lightly and go right back to "Grammy and Grampy." As long as they don't make derogatory remarks and show respect, I think you have to play it by ear. It's not always easy.I prefer professionals to call me by my first name, a lot ask you on their forms what you wish to be called. I always use my first name, as I think Mrs..... is too stuffy. To each their own, I guess.

Blame the names
on the 911 system. To help keep dispatchers from having to deal with sound alike names and sending emergency services to the wrong street the 911 center in most areas gets the final say so on whether a street name is okay or not.

When a small rural county I lived in went with the 911 system those of us who lived in the small cities had to rename some of the existing streets since there were streets in the rural areas of the county over 50 miles away with the same names.
Not street names, but how about....
the STUPID names they give the different models of new homes... When we bought ours, they were things like El Capitan, El Sonoro..... etc.  We bought the smallest and cheapest model and my DH always said that we bought the EL CHEAPO!

Used to work on Blue Rock STREET...lived with my folks on Blue Rock ROAD...which has about 3 blocks of what people think are Cheviot Road...goes back to Blue Rock Road....then 5 miles away, next street over is OLD Blue Rock Road Road....creates havoc for delivery people

We have Easy Street and Boardwalk in our subdivision.
I use Superpages to look names up.
Same as checking in a phonebook..no other personal info.
