The following dictation was dictated by me using Dragon Medical version 9.5. I have not yet proofed it.

>>>On the basis of a mildly low testosterone level, ranging from 213 to 308, coupled with symptoms of fatigue, muscle weakness, and erectile dysfunction, I decided to put him on testosterone replacement with a subsequent improvement noted in his symptoms. He is currently on AndroGel 7.5 grams (or three pumps) daily with good compliance. His total testosterone level is good at 368 on January 22, 2009. However, his PSA, which is known to be slightly elevated in the past, has increased to 4.5; it was 4.1 previously. ALT, hemoglobin, hematocrit, and lipid panel are normal. He denies any new symptoms.>>>
