I'm not sure if this is the correct board. I was wondering if anyone knows of a site that I can get a sample on a complaint that I would like to file to the administrator of a hospital. My mom has non-Hodgkin lymphoma and during her last 3 visits at the same hospital, they gave her the wrong medication. This is the medication that she has a severe reaction to. She has hallucinations, toxicity, etc. to this medication. She is in a different state and I would like to file a complaint to the administrator to prevent this from happening again, as she is in and out of the hospital. Thanks for the help.

how to file a health insurance complaint - any hints?
Background - I had to have radiation treatment for colon cancer and for the most part charged everything on my VISA.  I have all the VISA receipts.  Two office visits I wrote separate copay checks, which have cleared my bank.  The billing company for the oncologist says I still owe $20 copayment which I absolutely positively refuse to pay.  I have faxed to them a copy of all the VISA receipts and have spoken to the billing office and they have receipt of the two checks so they have receipts of all my visits/copays.  My insurance company says I only owe a single copay per visit so if the billing company says they have received the copayments then I have fulfilled my contract.  So who do I complain to?  It's bad enough to have to go through radiation, but now I'm getting burned again (LOL - punny punny, gotta laugh) because of their inadequate billing system.  Grrrrr.  I am so incredibly mad I feel like, go ahead, turn me over to a collection agency and MAKE MY DAY!!!!!  Anyone else go through this rigamarole/nightmare and who did you complain to?  I want people's bottoms to get waxed on this - I have spent literally hours on the phone trying to clear it up, not to mention the cost of mailing/faxing/etc.  Thanks in advance.

Being out of touch is a hazard of our job, but one reason you touched a nerve SM
that you should be aware of so you won't be surprised in future is that "ESL" is also code for bigotry, with in some sad cases the fussing over the genuine problems with achieving an accurate transcription just windowdressing for the real complaint.

Personally, I find the world and its people a fascinating place and love the way the Web has brought it to me in my little rural Georgia office, including the very wide range of attitudes here.
I've never taken a course and never had any formal
training, just kind of fell into it by accident and turned out I was good at it.   That was nearly 20 years ago though.  I know people who have gotten on-site positions in a doctor's office w/o having any schooling/prior experience.  If a company offers you a work at home position w/o the above I'd be very cautious. 
If no formal ed., would probably need mentor.

Never had any formal training here and never had a problem getting hired.
The only training I had was on the job. I started in a multi-physician OB practice, then went to the clinic pool when the docs sold out to the hospital clinic management, then into Hospital Radiology and finally acute care.
Black IS proper evening formal attire. Sounds gorgeous.

Rename your normal.dot file to something else then restart Word. It will create a new file.

Help..I lost my word file. How can I retreive a file that has been changed? sm

I was working on a large file and had saved it in Word. I then went back to work on the file and hit a key that made the page go blank.  When I went to close the document it asked me to save any changes on the document. I answered yes, and now when I click on that document it is all blank! Shouldnt the previous info saved be there, even if the current stuff is not? Please help me!! Thank you

This is my exact complaint

The looking up of addresses is ridiculous. I don't care HOW fast you type or HOW many macros and shortcuts you have, nothing will prepare you for that! The platform is user friendly where I work also. Most of the time with this company, I'm making less than $10 an hour because of this... and this job is my overtime job!


It's a small MTS and I really don't want to work for a national (from all that I hear on this board), so what does one do? How does one find the smaller companies to work for?


 


 


You are correct. If you have a complaint about something sm
they will tell you to go find another job. Communication with them is the pits.
Ridiculous complaint?

Busy, you should really try to be a little kinder.....or read her complaint a little more closely.  I think her complaint was not about CONSTRUCTIVE feedback.  If she gets hard copies she can't read, what good are they?  Just how ridiculous this QA thing can get; one of the editors in a company I worked for corrected my "40-year-old black female" to 40-year-old Black female."  No problem, if that's what they want, that's what they get.  I corrected my Expander to "Black."  Editor #2 comes along and corrects me saying "black or white" is never capitalized.  I fired back a very disgruntled e-mail and said editor #1 had corrected me and that I KNEW black was never capitalized but if they wanted it capitalized, fine with me but somebody needed to get their sh*t together and figure out what they wanted from me.  At least with that company, editor #2 had some sense, she sent back a very nice e-mail saying that to appease editor #1 to go ahead and capitalize it until I was off QA and then do it whichever way I wanted and in the meantime she would not count me off for it.  At least editor #2 had some sense but ridiculous?  YES, indeed. 


Busy, if you're working for both MQ and Spheris, I assume your experience is about the same as mine.  I worked for MQ and for Spheris when they were Edix.  Once I was off full edit (in a very short time), I never heard anything from either company QA again so I assume they were happy and I went right ahead doing my best.


Then it's already gone, why post a complaint?

would anyone else say that the biggest complaint as a Transcriptionist would be leg cramps and back pain?  and what suggestions would you give to help?
I ain't never heard this complaint before.

Complaint? What about a lawyer?
But why do you need a sample of a complaint? Write a letter explaining the details and mail it to the administrators of the hospital and copy it to the same people at the headquarter address (CEO). Ask for a response and mail it certified with a receipt requested.
Legitimate complaint
I think you are cheating yourself by not keeping a tally of the lost lines .. open a word document on your second screen and copy and paste these lines each time .. takes a few seconds and you need to be paid for what you are being asked to do .. otherwise, request a reprieve from this guy. Venting is one thing and it helps a little but cold hard cash actually speaks back to you!

This happens very often with abuse claims. A case is started, the claim is false, someone's reputation is wrecked, and it costs them a fortune in legal fees to restore it. If you are worried she is not getting help or advice, the only discrete way to find out is fake a spontaneous meeting and get into conversation with him/her and go from there. You could never let it be known that you read her/his records...records that she thinks are confidential. (Boy, are a lot of people in for a shock.)
My only complaint with mine is that...
my 2-year-old located the playboy channel (that I did not even know existed) in my car. Not such a big deal now, but I wish there were a way to disable it because someday, he won't be two anymore. Also, it taught him to scream the F word over and over and over again.

You can save the SH file in a text file and import to IT that way. sm
You may have to do some cleanup in the SH text file, but it sure would be faster than re-entering them all manually.
File taxes under the name on file w Social Security....
Name on W2 doesn't have to match, but your tax return has to match SS records. They don't want your marriage license or anything else. Did that for 13 years as instructed by IRS. Didn't have to change it with SS until state changed rules requiring drivers license to match SS records. Still use my maiden name in some limited circumstances.
She worked all those years with no complaint.

We are trying to dig out of the quagmire MQ/Philips has put us into.  Like I've said, MTing is no longer a profession or can be considered a "real job".  At best it is pickup work or sideline extra non-necessary money because one certainly can't depend on the money.

I called HR and I did not know you had to follow up with a written complaint. Is there an email
address for written complaints at MQ. The HR person sounded all disgusted and I am sure she has been getting tons of calls.
Filed complaint with Better Business Bureau against MTSO
for nonpayment after 90 days. Requested payment multiple times to no avail. Nothing but empty promises. Has anyone ever been successful getting paid after going through the BBB?

Don't do anything, just tell him to go ahead and do what he
has to do. If he's not for real, the threat has been not worked. If he is for real, he MUST have documentation to prove his case, or even then it's no use taking it to court. He will file in small claims probably, if less than $5000. He must prove his case, and you don't have to do anything, bottom line.
Don't get too ahead of yourself..
You may certainly have a valid argument, but don't base it on the "fact" that rad reports are all paid by report.  I have been doing rad for over 12 years now, and have yet to be paid by report.  Most are by the line, and any MTSO I have worked for, rad is one of the lowest line rates compared to other work type accounts.  Yes, there are companies that pay by the report, but, I surely wouldn't go so far as saying that is the "norm".

post it! You will feel so much better and be able to continue knowing you got your two cents in. Sometimes it makes you wonder about American standards for the practice of medicine, does it not? Doesn't it make sense that in America doctors should be able to communicate properly and have a handle on the language? Accents are not what I am talking about either. I am talking about vocabulary and grammar that can possibly cause a change outcome. This is a very serious business, medicine. Those who dictate should be qualified to dictate. Amen.

My record is around 45.  Right now i'm on a report that is up to about 23.  Talk about toxic!

If re-recording to digital, pretty easy. Play either in handheld or transcriber, and use either computer or digital handheld to capture. Just need the connecting cord between two units - can get at Radio Shack.
So how would you know ahead of time if

No one will laugh.  And, most likely no one will slam you for it either.  There are only a few really nasty, jealous people here.

Go ahead and pay for a decent program so you'll be set up. In all honesty (my opinion) you do not get the same kind of quality sound with these freebies as you do with the others. If you intend to do this job, at least get proper equipment.
